Clinical Manager (RN)- Home Health at Providence Health & Services in Greenacres, WA

In this position you will:
Provide clinical oversight to multidisciplinary team members for designated team with the support of a discipline supervisor as required for discipline-specific input. The clinical managers will interview and hire caregivers, as needed they will identify performance concerns and give timely feedback to direct reports; will develop and monitor improvement plans and deliver coaching and/or corrective actions. Will work closely with HR to investigate performance issues and terminate employment when necessary. Assign and/or provide consultative support for referrals as appropriate to clinicians based on clinical skill set and competency for a multidisciplinary team of clinical employees. Provide oversight and timely review of the Home Health caregivers OASIS/Plans of care/ documentation for delivery of home health services with goal of optimal patient outcomes and appropriate reimbursement by utilizing available resources. Also assist with the accurate and timely implementation of corrections to clinical records. Provide vertical support between Home Health clinical staff and leaders. Focusing these support efforts on fulfilling ministry and service line strategic goals including compiling data and/or reports utilizing both workflow and financial planning information. Regularly performs joint visits to ensure competency and holds meetings to discuss performance and development goals for direct reports. Assist with troubleshooting and triage of difficult to obtain orders. Oversight of communication, coordination and problem solving with physicians, referral sources, families and staff and payers. Maintain positive work environment with professionalism in all areas including decision making affecting others and in communication with both internal and external customers. Sets an example by modeling the Core Values and Behavior Standards and holds staff accountable for following them as well. Responsible for administration of client care services. Records and investigates all customer complaints and resolves issues when appropriate. Problem solves in a manner that considers the needs of customers and staff. Meets with staff as appropriate to address client complaints; works on a plan to resolve and address the complaint to promote a positive customer outcome; follows-up on staff performance as appropriate; documents process per policy. Responds to questions, concerns, or complaints from clients and other customers related to agency services and documents such interaction per agency policy/procedure. Provides a positive and supportive environment for customers. Maintain confidentiality of all patient demographics, including medical and financial information at all times. Maintains confidentiality of all company information. Ensures proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations. Coordinate Case Conferencing Schedules, Interdisciplinary Team (IDT) and meetings. Prepare and document as needed. Ensure Quality efforts and activities are communicated and incorporated into practice. Responsible follow-up for patient concerns and safety events - documenting in designated PSCS Quality system. Oversee and enforce standards of productivity, staffing continuity and utilization. May oversee coordination of admissions between Home Health Access and Home Health Scheduler by daily oversight of staffing functions to provide service to all new referrals. May triage phone calls from patients and patient's family members and provide direction to patients regarding their health care needs. Implement disaster/triage plan per protocol after hours, weekends and holidays as directed by management. Participate in rotation of after-hours administration call. Perform other work-related duties and special projects as assigned.
Qualifications:
Required qualifications for this position include:
Bachelor's Degree in health care related field Current and in good standing RN, PT, OT or SLP License in Washington State Current Driver's license and have access to daily use of an insured, reliable vehicle Current Provider CPR Certification 3 years experience in the nursing or rehab field Preferred qualifications for this position include:
Case Management experience Supervisory experience in Home Health
Salary Range:
$80K -- $100K
Minimum Qualification
Nursing, Healthcare ManagementEstimated Salary: $20 to $28 per hour based on qualifications.
